Girls 12U Softball – 2023

I do hereby, as the parent/guardian of the above named minor, authorize the treatment of such minor in the case of an injury or illness while under the supervision of the VVBASE organization. If in the event that reasonable attempts to contact me have been made unsuccessfully, I hereby give my consent for the administration of treatment or transportation to the nearest available hospital.

I as the parent/guardian agree to permit the above minor to participate with the team assigned under the VVBASE rules and regulations including the updated code of contact (provided above) for the current season. I also certify the age of the minor to be true and correct. I release VVBASE, Coaches, Germantown Park Board and Sponsors of any and all liability for injuries sustained to the above minor during participation of all activities or while in transit to or from such activities.
